Buying Skills

Many cards allow you to buy/learn skills on the fly. Pandora's box, Veteran's Guild, etc.
A) Can each skill be learned once? Or only one skill ever?
B) Do you have to have the skill card in hand to learn it, or can you use "proxies" or notations?

A) Pandora's Box is once ever and its gone, Veteran's Guild is in the FAQ and is once per hero per game for that location.

B) You have to have the cards in hand.
