Did some more work on this problem. The system will now log every forum/thread generation that takes over 10 seconds to complete along with some useful diagnostic information.
Let me know when the error happens next time and I will check the log.

that's gonna be a lengthly log, val. but we'll all keep our reporting up, i have no doubt. just let us know when you want us to stop. we don't want you going insane on us. 
what diagnostic information are you referring to, btw?

After analyzing the log and investigating further I think I may have finally found the cause. Or at least one of the causes.
Server CPU and RAM don't seem to be the issue, there are plenty of system resources available when the problem happens. It seems to be the database server slowing down, without overloading the CPU.
The cause turned out to be a poorly written query that was triggered on the post page to check if the user has made 20 posts in the last 24 hours. The query involved a table join on very large tables and used unindexed columns. The query took a long time to execute, slowing down the database server and all other requests being processed at the time. This also explains the problems people were experiencing with the post page.
I've rewritten the query to make it more efficient and tweaked the database configuration. Let's see what happens.

Checked this morning and there have been only 3 slow thread loads in the last 20 hours  As opposed to dozens / hundreds the day before.
In the mean time I optimized another inefficient join query in the thread display page. The database server load has gone down significantly as a result, in fact it is crazy to see that the database is now using less CPU than PHP!
Let's give it another week to be 100% sure that the problem is resolved. If so, I'll cancel the unnecessary RAM upgrade made a few months ago.

I've been watching the logs and the slow page generation issue is indeed resolved I've downgraded the RAM to what it was before and things are still running smoothly.
