theoretically, if you have shakgy the wyvern trainer, the hitpoint bonus could go to infinity; the answer to both parts of your question would be infinity

assuming its a paokai(120hp) and shakgy again, then at 40th level:
+80 specialty(+2hp/lvl)
+5 (2 rings vitality)
+2 vitality skill
+24(week of toughness)
+12(stamina skill)
+12(shrine that increases hp by 10%)
= +133hp for part a)

alright, I have to admit that I forgot about the Stamina Perk, but youīre right. Thatīs the solution for part a)
(by the way: itīs only +4 from the Rings of Vitality but thatīs fine).
So, does anybody have any idea for part b) or should ebbafan ask the next question?

= + 9 for goblin with 3 hp so %300?

actually, i made a mistake in my calculations: there is also an artifact set called "weapons of might" that add +2 hit points to all creatures involved, so the answer for part a) should be 135
and the 2 rings of vitality actually give you + 5 hit points after you get the other shrine/stamina perk/week of toughness perks(odd isn't it?)

Alright, I didnīt know that. Anyway, I think you should ask the next question especially considering the fact that you found a few things that I did not think about.
And the solution for part b):
The Gremlin (5 hp)
+ 13 (artifact with Artificer) according to the manual thatīs the max.
+ 5 (Rings of Vitality)
+ 2 (Vitality Perk)
+ 1 (Week of Toughness)
+ 1 (Shrine)
= +22 => +440 %
